$65 Initial program set-up fee for current patients
$85 Initial program set-up fee for new members
not seen in the past 3 months for a
Physical Therapy program
$130 Initial program set-up fee for individuals that
have never been seen for Physical Therapy
in our office
$65 Set-up fee for Physical Therapist to re-evaluate and update your program periodically
